Failure to: Provide sufficient support personnel to safely and effectively carry out the functions of the food and nutrition service.

Failure to: Provide appropriate treatment and care according to orders, resident’s preferences and goals.

Failure to: Ensure services provided by the nursing facility meet professional standards of quality.

Failure to: Safeguard resident-identifiable information and/or maintain medical records on each resident that are in accordance with accepted professional standards.

Failure to: Give the resident's representative the ability to exercise the resident's rights.

Failure to: Provide appropriate pressure ulcer care and prevent new ulcers from developing.

Failure to: Assist a resident in gaining access to vision and hearing services.

Failure to: Provide activities to meet all resident's needs.

Failure to: Have the Quality Assessment and Assurance group have the required members and meet at least quarterly

Failure to: Safeguard resident-identifiable information and/or maintain medical records on each resident that are in accordance with accepted professional standards.

Failure to: Ensure drugs and biologicals used in the facility are labeled in accordance with currently accepted professional principles; and all drugs and biologicals must be stored in locked compartments, separately locked, compartments for controlled drugs.

Failure to: Ensure each resident’s drug regimen must be free from unnecessary drugs.

Failure to: Provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift.

Failure to: Provide safe, appropriate pain management for a resident who requires such services.

Failure to: Provide appropriate treatment and care according to orders, resident’s preferences and goals.

Failure to: Provide care and assistance to perform activities of daily living for any resident who is unable.

Failure to: Honor the resident's right to request, refuse, and/or discontinue treatment, to participate in or refuse to participate in experimental research, and to formulate an advance directive.
