Failure to: Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.

Failure to: Not transfer or discharge a resident without an adequate reason; and must provide documentation and convey specific information when a resident is transferred or discharged.

Failure to: Develop, implement, and/or maintain an effective training program for all new and existing staff members.

Failure to: Set up an ongoing quality assessment and assurance group to review quality deficiencies and develop corrective plans of action.

Failure to: Safeguard resident-identifiable information and/or maintain medical records on each resident that are in accordance with accepted professional standards.

Failure to: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.

Failure to: Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature.

Failure to: Provide routine and 24-hour emergency dental care for each resident.

Failure to: Ensure each resident’s drug regimen must be free from unnecessary drugs.

Failure to: Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full time basis.

Failure to: Provide enough nursing staff every day to meet the needs of every resident; and have a licensed nurse in charge on each shift.

Failure to: Provide activities to meet all resident's needs.

Failure to: Provide care and assistance to perform activities of daily living for any resident who is unable.

Failure to: Ensure residents do not lose the ability to perform activities of daily living unless there is a medical reason.

Failure to: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.

Failure to: Ensure services provided by the nursing facility meet professional standards of quality.

Failure to: Provide safe and appropriate respiratory care for a resident when needed.

Failure to: Protect each resident from all types of abuse such as physical, mental, sexual abuse, physical punishment, and neglect by anybody.

Failure to: Include as part of its infection prevention and control program, mandatory training that includes written standards, policies, and procedures for the program.

Failure to: Give their staff education on dementia care, and what abuse, neglect, and exploitation are; and how to report abuse, neglect, and exploitation.

Failure to: Ensure that staff members are educated on resident rights and facility responsibilities to properly care for its residents.

Failure to: Designate a qualified infection preventionist to be responsible for the infection prevent and control program in the nursing home.

Failure to: Ensure medication error rates are not 5 percent or greater.

Failure to: Implement gradual dose reductions(GDR) and non-pharmacological interventions, unless contraindicated, prior to initiating or instead of continuing psychotropic medication; and PRN orders for psychotropic medications are only used when the medication is necessary and PRN use is limited.

Failure to: Report COVID19 data to residents and families.
